Outpatient Physical Therapist
Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to the patient's needs, goals, and physician direction. This role is responsible for delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective community re-engagement. The therapist also serves as an educator for patients, families, students, colleagues, and themselves.
Responsibilities
Complete comprehensive evaluations of assigned patients.
Utilize clinical reasoning to develop appropriate treatment plans and goals focused on progress towards discharge.
Document medical records clearly, accurately, and promptly, including goals, assessments, and charge entries.
Ensure care provision under signed, valid physician orders and within insurance approval.
Participate in student programs by providing a positive learning environment and appropriate supervision.
Develop an individualized professional development plan and pursue continuing education opportunities.
Adhere to relevant State Practice Acts and maintain professional licensure in good standing.
Understand and enact the organization's performance improvement initiatives and action plans.
Required Experience / Certifications / Licensure Degree in Physical Therapy from an accredited school.
Minimum of 1 year of outpatient clinical experience.
Current CPR certification.
Schedule
This is a 13-week contract position with a 5x8.0 schedule.
