Job description
Qualified Physical Therapist
CVHCare is looking for a qualified Physical Therapist to provide therapy services to patients in the designated service areas. The Physical Therapist will be responsible for conducting evaluations, developing treatment plans, and implementing therapy interventions consistent with the highest professional standards.
Location
Danville, CA and surrounding areas
Assignment Duration
This is a 13-week contract position.
Schedule
Day shift; additional travel outside the assigned area and weekend coverage may be required.
Job Description
CVHCare is looking for a qualified Physical Therapist to provide therapy services to patients in the designated service areas. The Physical Therapist will be responsible for conducting evaluations, developing treatment plans, and implementing therapy interventions consistent with the highest professional standards.
Responsibilities
Conduct OASIS assessments and evaluations for patients.
Develop and implement individualized treatment plans.
Provide routine therapy sessions and assess patient progress.
Document therapy sessions and patient interactions thoroughly.
Collaborate with interdisciplinary teams to enhance patient care.
Required Experience / Certifications / Licensure
Current Physical Therapist license in California.
Current BLS certification.
Valid driver's license and auto insurance.
Annual TB test and immunizations (MMR, Varicella, HepB, TDap, Flu).
Successful completion of a drug screening and physical examination.
