Job Description
Home Health Physical Therapist/Occupational Therapist/Speech-Language Pathologist
Assignment Duration
This is a 13-week contract position.
Schedule
Monday to Friday, 8-hour shifts.
Job Description
We are seeking experienced Home Health Physical Therapists, Occupational Therapists, and Speech-Language Pathologists to provide high-quality care in Lehigh and Northampton counties. Candidates may be asked to assist in other territories within a 60-mile radius of the facility address as needed.
Responsibilities
Conduct patient evaluations and assessments.
Implement and modify treatment plans based on patient needs.
Document patient progress in Homecare Homebase and OASIS.
Coordinate with the interdisciplinary team for comprehensive patient care.
Manage a productivity benchmark of 28-32 points per week, handling 4-6 visits per day based on visit type.
Required Experience / Certifications / Licensure Home Health and OASIS experience is required, with no exceptions.
Experience with Homecare Homebase documentation required.
Contract may consider candidates without HCHB experience at a reduced rate for the first two weeks if the manager is willing to provide training.
