Job Responsibilities

Conduct patient evaluations in both inpatient and outpatient settings and create tailored treatment plans

Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function

Engage with nurses, physicians, case managers, and other rehab professionals to maximize patient outcomes

Guide patients and their families in understanding therapeutic objectives, discharge planning, and home exercises

Maintain accurate documentation of evaluations, treatments, and patient progress in line with facility standards and regulations

Join care team meetings and interdisciplinary conferences to promote a cohesive care approach

Education and Certifications

Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

State licensure as a Physical Therapist

CPR certification preferred

Specialized certifications, like neurological or geriatric, are advantageous though not mandatory

Skills

Preferably possess prior experience in the field

Capability to assist patients facing complex medical and mobility challenges
