nCoverage area: Gaston County
nSchedule: Monday-Friday, 8-5pm

nResponsibilities
nAs a Home Health Physical Therapist , You w ill:
nnnP erform physical therapy evaluation and treatment of patients based upon physician orders, including recommendation and procurement of adaptive equipment
n
nnEstablish measurable goals and develop treatment plans to move patients toward their maximum level of independence and/or function
n
nnMaintain communication among the treatment team, physician, patient, and family members as applicable
n
nnMaintain thorough and timely documentation of services provided to ensure proper billing
n
nnSupervise Physical Therapist Assistants and Physical Therapy Aides in direct patient care and patient-related activities in accordance with state practice act
n
n nQualifications
nnnMinimum of Bachelors Degree in Physical Therapy from a college or university with accredited Physical Therapy program
n
nnNew physical therapy graduates may be considered in select markets based on program availability
n
nnAt least (2) years of appropriate experience as a physical therapist. Community/hospice experience is preferred
n
nnValid license form the state of practice
n
nnPossess and maintains current CPR certification
n
