Job Responsibilities

Conduct thorough evaluations in both inpatient and outpatient settings and tailor personalized treatment plans for each patient

Implement effective physical therapy treatments aimed at improving mobility, strength, balance, and overall functionality

Work alongside a multidisciplinary team including nurses, physicians, and case managers to enhance patient outcomes

Educate patients and their families about therapeutic goals, discharge processes, and home exercise routines

Maintain accurate documentation of evaluations, treatment plans, and patient progress in compliance with facility standards

Engage in interdisciplinary team meetings to ensure seamless patient care and planning

Education and Certifications

Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution

State licensure as a Physical Therapist

CPR certification is advantageous

Additional specialized certifications (like neurological or geriatric) are welcomed but not mandatory

Skills

Preferred candidates will have relevant practical experience

Ability to address complex medical and mobility challenges

Exceptional communication and teamwork abilities

Familiarity with digital charting and clinical documentation
