Description
Assess patients in either an inpatient or outpatient setting and formulate personalized treatment plans
Implement physical therapy interventions centered around mobility, strength, balance, and functionality
Engage with nurses, physicians, case managers, and fellow rehab professionals to maximize recovery outcomes
Inform patients and their families about therapeutic objectives, discharge planning, and at-home exercises
Record evaluations, treatments, and patient progress following facility and regulatory protocols
Partake in care team discussions and interdisciplinary meetings to ensure cohesive care
Education
Doctorate in Physical Therapy (DPT) or equivalent degree from a recognized program
Certifications
State licensure as a Physical Therapist
CPR certification preferred
Specialty certifications (for example, neurological, geriatric) are advantageous but not mandatory
Skills
Prior experience is highly preferred
Capability to assist patients with complex medical and mobility challenges
Excellent communication and teamwork abilities
Comfortable utilizing digital charting and clinical documentation tools
