Description

Conduct evaluations for patients in inpatient or outpatient settings and craft personalized treatment plans

Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function

Collaborate closely with clinical staff including nurses, physicians, and case managers to optimize patient outcomes

Guide patients and their families on therapeutic objectives, discharge strategies, and necessary home exercises

Maintain detailed records of evaluations, treatments, and progress in compliance with facility policies and regulations

Engage in interdisciplinary team rounds and conferences to ensure unified care delivery

Education

Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

Current state licensure as a Physical Therapist

CPR certification is preferred

Additional specialized certifications (e.g., neurological, geriatric) are advantageous, yet not mandatory

Skills

Prior experience is highly advantageous

Capability to manage patients with intricate medical and mobility challenges

Exceptional communication and teamwork abilities

Familiarity with digital charting and clinical documentation processes is essential
