Job description
Physical Therapist
Evaluate residents’ mobility, strength, balance, gait, and functional limitations through initial and ongoing assessments. Develop and implement individualized, person-centered treatment plans to restore or improve physical function, reduce pain, and prevent disability. Provide direct skilled physical therapy interventions, including therapeutic exercises, gait and balance training, and use of physical modalities. Monitor resident progress and adjust treatment plans as needed to achieve therapy goals. Participate actively in interdisciplinary care team meetings and assist with discharge planning. Educate residents, families, and caregivers on home exercise programs and safety techniques to promote independence. Supervise and support Physical Therapist Assistants (PTAs) and therapy aides in accordance with licensure regulations. Accurately document treatment sessions, progress notes, and billing information in compliance with facility, state, and federal standards. Ensure adherence to all relevant documentation, practice, and regulatory requirements. Support quality assurance and performance improvement initiatives within the rehabilitation team. Work collaboratively in a skilled nursing facility/long-term care environment, which may involve frequent standing, bending, lifting, and assisting residents with mobility.
