Description

Conduct thorough evaluations of patients, whether in inpatient or outpatient scenarios, and craft tailored treatment plans that cater to individual needs

Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall functionality

Collaborate with a multidisciplinary team of nurses, physicians, case managers, and rehabilitation professionals to maximize patient outcomes

Educate patients and their families regarding therapeutic objectives, discharge strategies, and at-home exercise routines

Maintain precise documentation of evaluations, therapies, and ongoing progress in line with institutional and regulatory standards

Engage in interdisciplinary discussions and rounds to ensure cohesive and integrated care for all patients

Education

Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

State licensure as a Physical Therapist

CPR certification preferred

Specialty certifications (e.g., neurological, geriatric) are advantageous but not essential

Skills

Previous clinical experience highly desirable

Adeptness to manage patients with complex mobility and medical challenges

Exemplary communication and teamwork capabilities

Proficiency in digital record-keeping and clinical documentation systems
