Job description
Flagstar Rehab is Seeking Physical Therapists (PT) for Skilled Nursing Facilities!
Flagstar Rehab puts Physical Therapists in the driver's seat when it comes to managing their careers and work-life balance. Pay Rate: $46-$48 Hourly (W-2), $57-$60 Hourly (1099)
Physical Therapist Requirements and Responsibilities:
Current NYS PT License and Registration
Valid BLS Certification
EMR Competence
Meets all State and Federal requirements for US employment in a Nursing Facility
Malpractice/Liability Insurance (if applicable)
Provide efficient evaluations and create individualized treatment plans for PTAs to follow under your supervision.
Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
Monitor and document patient progress with the use of innovative EMR software.
