Description
Assess patients in both inpatient and outpatient environments, designing tailored rehabilitation programs specific to their needs
Deliver physical therapy techniques aimed at enhancing mobility, strength, balance, and overall functional capacity
Work cohesively with a multidisciplinary team including nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes
Guide patients and their families through therapeutic aims, planning for discharge, and homework assignment exercises
Maintain precise documentation of assessments, treatments, and progress to comply with institutional and regulatory standards
Engage in collaborative care discussions and interdisciplinary meetings to guarantee integrated patient care
Requirements
Doctor of Physical Therapy (DPT) or equivalent credential from an accredited institution
Certifications
Valid state license to practice as a Physical Therapist
Current CPR certification is preferred
Additional specialized certifications (like neuro or geriatrics) are advantageous but not mandatory
Skills
Prior clinical experience is highly preferred
Proficient in managing patients with intricate medical and mobility challenges
Excellent communication abilities and a knack for teamwork
Familiarity with digital charting and clinical documentation is necessary
