Description
Conduct thorough evaluations of patients in both inpatient and outpatient environments, creating personalized treatment strategies
Implement physical therapy techniques targeted towards mobility, strength, balance, and functional improvement
Work alongside nurses, physicians, case managers, and other rehabilitation specialists to achieve the best patient outcomes
Educate patients and their families about therapeutic objectives, discharge strategies, and home exercise plans
Maintain detailed documentation of assessments, treatments, and progress inline with facility and regulatory standards
Engage in interdisciplinary care team meetings and discussions to ensure seamless patient care
Education
Doctorate in Physical Therapy (DPT) or an equivalent credential from an accredited institution
Certifications
Valid state licensure as a PT / Rehabilitation Therapist
CPR certification is preferred
Special certifications (e.g., neurological, geriatric) would be advantageous but are not mandatory
Skills
Prior experience is strongly desired
Ability to manage patients with multifaceted medical and mobility challenges
Excellent communication and collaboration skills
Proficient in utilizing digital charting and clinical documentation tools
