Job description
Are you a Respiratory Therapist who is passionate about improving resident outcomes, preventing avoidable hospitalizations, and making a lasting impact in long term care? We are seeking a Pulmonary Care Coordinator to lead and support our facility's pulmonary program through clinical excellence, interdisciplinary collaboration, education, and strong communication.
nPulmonary Care Coordinator Essential Responsibilities
nnnComplete pulmonary assessments and evaluations for residents enrolled in the pulmonary program
n
nnMonitor respiratory status and identify changes in condition requiring intervention
n
nnDevelop, implement, and update individualized pulmonary care plans
n
nnCoordinator respiratory interventions
n
nnServe as a primary pulmonary resource and communication leader for the facility
n
nnProvide on-going education, coaching, and support to nursing staff, therapy staff, and respiratory personnel
n
n nPulmonary Care Coordinator
nRequired Qualifications
nnnCurrent Registered Respiratory Therapist RRT license in good standing
n
nnCurrent BLS certification
nPreferred Qualifications
n
nnExperience in skilled nursing, long-term care, rehabilitation, or post-acute care
n
nnLeadership, supervisory, or program development experience
n
n
