Job description
HealthCare Partners, MSO is seeking a Quality Interventions Pharmacist in Garden City, NY to optimize medication use and improve quality performance across Medicare, Medicaid, and Commercial lines. The pharmacist will drive measure improvement, conduct high-volume outreach, and educate providers and staff on guidelines.
The role requires PharmD, CRC certification, and experience in medication adherence and quality measures; field visits may be involved, with a strong emphasis on collaboration
