Job description
Bluebonnet Trails Community Services’ Youth Crisis Outreach Team (YCOT) provides crisis intervention, assessment, intensive case management, relapse prevention, and care coordination for youth in crisis.
The Youth Outreach Specialist delivers quick, in-person responses, with up to 90 days of transitional services to each youth and family, working with an interdisciplinary team of clinicians to stabilize and link to community supports.
