Job description
St. Gregory Recovery Center in Bayard, IA is seeking a part-time Case Manager to join the treatment team. The role coordinates care across providers, develops discharge and aftercare plans, and documents progress in the client’s clinical records.
Responsibilities include engaging with clients and families, coordinating referrals, and ensuring services on the ISP are implemented in a timely manner. On-site work supported by a collaborative team.
