Job description
Community Medical Services (CMS) is seeking a Community Impact Specialist to foster relationships with community partners and support coordination of care for patients recovering from addictions. You’ll engage with coalitions, health homes, hospitals, and emergency services, documenting encounters and facilitating access to resources.
The role emphasizes outreach, data maintenance in multiple databases, and crisis support, with travel to CMS locations as needed.
