Job description

MISSION STATEMENT:

ROLES AND RESPONSIBILITIES

Maintains a relationship with all partnering agencies and referral sources.

Efficiently document all billable interactions into AKAIMS, utilizing DAP note formatting.

Coordinates with referral sources, involved family members, community agencies and other treatment providers to provide the best treatment and case management options for the individual participant's needs.

Services a caseload of active participants, all of which will be adults returning from incarceration and entering into the community.

Provides motivational interviewing, social, and emotional support to participants entering and engaging in the program.

Ability to navigate multiple grants and organization programs.

Create crisis stabilization plans as needed for participants exploring treatment and coordinate assistance from peer supports to accomplish immediate/crisis goals.

Assists participants in identifying case management services; as needed, in accordance to best practices to provide linkage and advocacy that participants need. To include coordinating referrals that pertain to employment, mental health, housing, medical with local community clinics, social service agencies, continuing care and provide follow-up on all referrals made.

Works with participants to develop and update personalized plans prior to program completion to ensure participants leave the program with a clear plan for continued housing, employment, support, and treatment needs.

Able to pass a background to enter into correctional facilities and participate in InReach.

Assist participants in exploring available grants to aid them in getting funding for housing, transportation, education, and other needed items.

Attends all relevant training.

Attends Fairbanks reentry coalition meetings and case management meetings. Ability to provide client updates in meetings.

Attends staff meetings and works collaboratively with other staff in case staffing/review.

Transport participants on assigned activities, case management services and outings as needed.

Maintains a respectful relationship with participants, former participants, family members and peers.

Performs other duties as needed and/or assigned.

QUALIFICATIONS AND EDUCATION REQUIREMENTS An associate's degree in human services or related field is preferred. An equivalent combination of education and experience may be accepted.

If in recovery, one year of continuous sobriety prior to hire and active and continuing participation in a recovery support program, such as a 12-step program.

Must obtain CDC II OR CDC I & NACA I within 3 years of hire.

Valid Alaska driver's license and insurable under TRUE NORTH RECOVERY INC's automotive insurance, which requires a driver to be at least 21 years of age.

Continued employment is contingent upon receipt of a satisfactory state and federal background check.

PREFERRED SKILLS Excellent verbal and written communication skills.

Proficient with PCs and standard office equipment.

Ability to make decisions and exercise good judgment.

Ability to work independently with minimal supervision.

Ability to work under pressure and prioritize workload.

Strong public speaking and organizational skills.

Strong problem-solving skills.

Observes participants rights to confidentiality.

Ability to coordinate multiple activities.

Equivalent combination of HSD or GED and experience may be accepted.

PHYSICAL REQUIREMENTS

This role is primarily performed in an office building with standard office environment and controlled temperatures. When working outdoors or at a job site the employee may be exposed to construction equipment or other moving objects/equipment; reference job site specifics. The employee is occasionally required to lift and/or move up to 50 lbs. Employee is frequently required to sit, stand, walk, talk/hear, see, read, and personally interact with others.
