VitalCore Health Strategies is seeking a Reentry Specialist (with SUD experience) for the Howard R. Young Correctional Institution in Wilmington, Delaware.
This full-time role focuses on connecting offenders to community programs, ensuring continuity of care after release, and advocating for patient needs across multiple systems. You will work within a multidisciplinary team to support individuals with mental health issues, co-occurring substance use disorders, and chronic medical conditions.
