Job description
EvergreenHealth in Washington is seeking a Referral Intake Nurse to initiate, screen and triage Home Health and Hospice referrals. You will maintain relationships with referral sources and insurers, and educate sources on eligibility and services.
The role oversees referrals handled by Intake Support Specialist and requires a WA nursing license, BLS certification, and 2 years of Home Health/Hospice experience. A Master’s in nursing is preferred. Per diem staff benefits apply.
