Job description
Continuum Therapy Partners seeks a Director of Rehab to guide therapists across facilities within a region, leading physical, occupational, and speech therapy teams to ensure cohesive, high‑quality care.
You will oversee program development, coordinate referrals, schedule and direct rehabilitation services, and foster collaboration between Continuum staff and facility leadership with a focus on resident outcomes.
