Job description
A healthcare organization in Vermont is looking for a Registered Dietitian for a full-time position supporting both long-term care and inpatient services.
Position Highlights Approximately 40% long-term care and 50% inpatient , with 10% focused on quality improvement and other projects
Conduct comprehensive nutrition assessments for patients at nutritional risk
Develop and oversee individualized nutrition and hydration plans
Provide nutrition education to patients and families
Participate in care conferences, meal rounding, and quality improvement initiatives
Collaborate with physicians and multidisciplinary clinical teams
What We re Looking For Registered Dietitian
Comfortable working in both long-term care and inpatient settings
Knowledge of evidence-based nutrition practices
Ability to assess residents for weight changes, wounds, and other nutritional concerns
Strong documentation and electronic medical record skills
Ability to support regulatory compliance and survey readiness
