Job description
Riverside Health System in Newport News, VA is seeking a Registered Nurse to assess PORT participants and coordinate care across settings under the supervision of the practice manager and director.
You will collaborate with a multidisciplinary team, document care plans, educate patients and families, and ensure safe transitions. Weekend and irregular shifts are required; compensation includes a competitive hourly range.
