nHybrid: Candidate must be able to reach the office within an hour 3221 McKelvey Road, Bridgeton, MO
nSchedule: Monday-Friday, 8:00 AM - 4:30 PM during training. After training, flexible hours of 7:30 AM - 4:00 PM or 8:30 AM - 5:00 PM are allowed.

nJob Summary:
nUtilizes skilled intervention, clinical best practice knowledge and collaboration with the patient, physician and multidisciplinary treatment team to pace care, achieve quality outcomes and negotiate appropriate reimbursement. Ensures the timely and seamless clinical transition of patients through medically appropriate levels of care.
nJob Responsibilities and Requirements:
nPRIMARY RESPONSIBILITIES
nnnReviews all patients identified for review to determine the medical necessity and appropriateness of enrollment in the care management program.
n
nnAssesses all assigned patients and integrates family information from social work to collaboratively determine potential, actual and the next level of care needed.
n
nnHolds, interprets, and integrates the patient's story into the overall multidisciplinary plan of care.
n
nnCoordinates and facilitates access to services and patient care progression using best practice interventions that will produce favorable patient outcomes.
n
nnCollaborates with physicians, nursing, social work, and multiple disciplines, departments, payers, and agencies to eliminate barriers to efficient delivery of care in the appropriate setting.
n
nnActively participates in clinical performance improvement activities as assigned.
n
nnBuilds a network of positive working relationships that advocate for the patient.
n
nnDocuments clinical activity in the medical record and supports and coaches accurate clinical documentation by physicians and others.
n
nnPursues continuing education in area of expertise/leadership.
n
nnWorks in a constant state of alertness and safe manner.
n
nnPerforms other duties as assigned.
n
n nEDUCATION
nnGraduate of accredited school of nursing or education equivalency for licensing
n nEXPERIENCE
nnTwo years' registered nurse experience
n nPHYSICAL REQUIREMENTS
nnnFrequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.
n
nnFrequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
n
nnFrequent use of vision and depth perception for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.
n
nnFrequent use of hearing and speech to share information through oral communication. Ability to hear alarms, malfunctioning machinery, etc.
n
nnFrequent keyboard use/data entry.
n
nnOccasional bending, stooping, kneeling, squatting, twisting and gripping.
n
nnOccasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.
n
nnRare climbing.
n
