nAssess & Plan
nComplete in-home RN assessments; craft individualized Care Plans with client/family approval.
nCoach & Supervise
nOrient caregivers to each Care Plan, perform field competency checks, and ensure ongoing compliance.
nMonitor & Document
nTrack vitals, condition changes, and interventions in our HIPAA-secure EMR; escalate concerns promptly.
nCollaborate & Advocate
nCoordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
nQuality & Compliance
nMaintain audit-ready client files, collect initial service payments, and uphold state home-care regulations.
nWhat You’ll Bring
nActive Registered Nurse (RN) license
nBachelor’s in Nursing or related human-services field – OR – 2+ years’ geriatric/home-care experience
nSolid clinical assessment skills and a passion for problem-solving
nCPR/First Aid certification and a valid driver’s license
nWarm, professional communication that builds trust with families and field staff
