Baptist Health is looking for a Registered Nurse, Case Manager at Baptist Medical Jacksonville - Downtown campus. This is a Full Time Day shift opportunity working Monday - Friday in the In-Patient Case Management Department at Baptist Jacksonville - Downtown campus.
Case Manager, Job Responsibilities
Care coordination for hospitalized patients.
High risk screenings and assessments.
Coordinating post-acute care services with proper documentation.
Collaborating with multidisciplinary team members.
Experience Requirements
1 year Case Management experience
Education & Credential Requirements
Associate's Degree Nursing (ADN)
Basic Life Support (BLS)
Driver's Licensed (DMV)
FL Active licensed Registered Nurse (RN)
