Job description
Houston Methodist is seeking a Registered Nurse Case Manager to coordinate complex patient care across the continuum in a mission-driven, patient-centered environment. In this role, you will perform comprehensive assessments, develop individualized care plans, and collaborate closely with physicians, interdisciplinary teams, patients, and families to promote safe transitions, reduce readmissions, and optimize outcomes. You’ll leverage EMR tools, evidence-based guidelines, and community resources while contributing to a culture of safety, innovation, and continuous learning within a nationally recognized health system.
