Registered Nurse Case Manager – End-of-Life Care
Job Title: RN Case Manager – Dignity. Compassion. Clinical Excellence.
Pay: $90,000 – $130,000 DOE
Benefits: Full Benefits Package
Location: Greater Twin Cities Metro Area

What the Role Involves
Supporting patients through complex emotional and physical transitions
Providing education and reassurance to families
Coordinating a high-quality care experience from admission through bereavement
Collaborating with social workers, aides, and medical directors
We’re Looking For Someone Who
Holds an RN license in MN
Has prior hospice, oncology, or palliative experience
Balances heart-centered care with medical skill
Understands the sacredness of this work and approaches it with grace
