Job description
UEW Health is seeking a qualified Registered Nurse Case Manager to join our compassionate team. You will assess patient needs, coordinate care, and ensure clear communication among the care team and with patients and families.
Qualified candidates hold an active RN license, with at least one year of home health or related experience, strong assessment skills, and excellent organizational abilities. Proficiency with computers and medical equipment is required.
