Job description
Manchester Memorial Hospital, part of Eastern Connecticut Health Network, seeks a Registered Nurse Case Manager to coordinate patient care across the continuum in our acute-care community hospital. In this role, you will perform comprehensive assessments, develop individualized care plans, and collaborate with multidisciplinary teams to ensure safe, efficient, and patient-centered care. You will support discharge planning, patient and family education, and connection to community resources while promoting quality, reducing readmissions, and advancing a culture of compassion, teamwork, and continuous learning.
