Job description
Hartford Healthcare is seeking an RN Case Coordinator to join our Healthcare & Medical Services team. In this role, you will coordinate care across the continuum, develop and monitor patient-centered care plans, and facilitate safe transitions between inpatient, outpatient, and community settings.
