Role Overview:
This position involves assessing, developing, and implementing discharge plans for patients by collaborating with an interdisciplinary team. The nurse ensures compliance with discharge planning standards, identifies barriers to patient care, and manages cases at high risk for readmission. Responsibilities include participating in utilization management, acting as a liaison with post-acute providers, and applying evidence-based practices tailored to patient needs. Qualifications:
