Job description
Manchester Memorial Hospital, part of Eastern Connecticut Health Network, seeks a Registered Nurse Case Manager to coordinate patient-centered care for diverse medical-surgical and specialty populations. In this role, you’ll complete comprehensive assessments, develop and manage individualized care plans, and collaborate closely with physicians, therapists, social workers, and community partners to ensure safe, timely transitions of care. You’ll provide patient and family education, advocate for resources, support quality and utilization initiatives, and document in the EMR while working in a compassionate, team-focused environment.
