Hebrew SeniorLife, a Boston-based nonprofit affiliated with Harvard Medical School, is seeking a Hospice Registered Nurse Case Manager to provide compassionate, patient-centered end-of-life care for older adults. In this role, you will coordinate interdisciplinary care plans, conduct in-home and facility visits, manage symptoms, educate patients and families, and advocate for goals of care.
