Job Title: Registered Nurse Field Assessor (FA)
Location: Chicago, IL
Compensation: $90-$100 per visit
This Position Is Not Eligible For Benefits.
As a Registered Nurse Field Assessor (FA), you will be completing Long Term Care insurance assessments. These hands-off assessments require you to obtain claimant demographics, physician information, a current medication list, and evaluate the claimant’s functional level of independence.
The assessment process also involves completing a brief cognitive screening and evaluating the claimant’s ability to complete ADL and IADL tasks. The ideal candidate will be technologically savvy and able to use a point of care web application/system on a laptop or tablet, in real-time for documentation purposes.
Responsibilities:
Must demonstrate good clinical reasoning and judgment as it relates to assessing ADL performance, cognition, and home safety.
Must be comfortable with the use of point of care technology when completing assessments.
Education and Experience:
Current, unencumbered RN licensure in the state of practice.
Minimum of 2 years of home health experience or 2 years nursing experience in an equivalent area of practice serving the older adult population.
Has, or be willing to obtain, a professional liability insurance policy.
Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
