Job description
Hartford Healthcare seeks a compassionate Hospice Registered Nurse (Visiting Nurse) to provide in-home end-of-life care across our Connecticut service area. In this role, you will conduct comprehensive patient assessments, manage complex symptoms, administer medications, and develop individualized care plans in collaboration with an interdisciplinary hospice team. You will educate and support patients and families, coordinate services with physicians and community resources, and ensure compliance with hospice standards.
