Job description

Site: Mass General Brigham Home Care, Inc.

Mass General Brigham relies on a wide range of professionals, including doctors, nurses, business people, tech experts, researchers, and systems analysts to advance our mission. As a not-for-profit, we support patient care, research, teaching, and community service, striving to provide exceptional care. We believe that high-performing teams drive groundbreaking medical discoveries and invite all applicants to join us and experience what it means to be part of Mass General Brigham.

Job Summary

The position to which you are applying is represented by a collective bargaining unit, Massachusetts Nurses Association.

Reimagine what nursing can look like.

At Mass General Brigham Home Care, you'll build meaningful relationships, use your clinical judgment, and make a difference where patients feel most at home.

As an RN, you'll have the independence to manage your patients and schedule, without giving up the support of a strong team.

Join our Logan team

Most visits will be in East Boston and Winthrop, with additional coverage in Chelsea and Revere.

You'll work with a diverse patient population and provide care where patients live every day.

Why you'll like this team

You have independence, but you're never on your own.

*A schedule that works for you: Full-time, Monday-Friday with no evenings, 5-6 visits per day, and the autonomy to manage your own schedule and appointments.

*Smaller geography: A focused service area means less time traveling between visits, more time available for patient care, and convenient access to the office when you need to replenish supplies. The territory is also conveniently located near the hospital and other nearby locations for dropping off labs. City parking passes are provided for new team members.

*Dedicated Admission RN support: Dedicated Admission Registered Nurses support new patients coming into the service line and help establish their plan of care, allowing you to focus more of your time on ongoing patient visits.

*Ancillary support services: Scheduling coordinators, liaisons, and other support services help with the administrative and coordination needs of care, allowing you to spend more time focused on your patients.

*Strong team culture: Work alongside a collaborative team that values communication, teamwork, and supporting one another. Weekly interdisciplinary pod meetings, monthly team meetings, and quarterly in-person team days help keep the team connected.

*Support to grow: Ongoing training, education, and opportunities to collaborate across disciplines.

A strong start matters

Home care can be a big transition, especially if you're coming from a hospital or other clinical setting. Our onboarding is designed to help you feel prepared and confident working independently in the community. You'll also have ongoing opportunities for training, education, and professional development.

Your time matters, too

Your schedule includes dedicated time for documentation, chart review, and care coordination. Our MGB electronic documentation system gives you easy access to doctors' notes and orders, helping make communication and care coordination more seamless.

Your safety matters

We provide safety resources to support you in the field, including:

*GPS-enabled safety tools

*Real-time alert features

*Security-supported key fob access

What you'll do

As an RN, you'll oversee the patient's plan of care and work closely with an interdisciplinary team.

*Complete patient assessments and admissions

*Develop and manage individualized care plans

*Provide skilled nursing care, education, and treatment

*Coordinate care with patients, families, and providers

*Document care and review charts

*Collaborate with the interdisciplinary team

*Deliver high-quality, patient-centered care

Qualifications

What you'll bring

Graduate of an approved School of Nursing

Current Massachusetts Registered Nurse license

At least one year of acute care nursing experience (home care experience preferred)

Comfort working with various computer systems; experience with handheld clinical devices preferred
