Job description
About the Role: Join Rising Star Home Care Services as a Registered Nurse (RN) specializing in Home Care Client Assessments. This per diem position offers the flexibility to provide compassionate care in the comfort of our clients' homes in Elkton, MD.
nResponsibilities:
nConduct comprehensive assessments of clients' health and home care needs.
nDevelop and implement individualized care plans in collaboration with clients and families.
nMonitor patient progress and adjust care plans as necessary.
nProvide education and support to clients and their families regarding health management.
nDocument assessments, care plans, and patient interactions accurately and timely.
nCoordinate with other healthcare professionals to ensure quality care delivery.
nAdhere to all safety and health regulations while providing care.
nParticipate in ongoing training and development opportunities.
nRequirements:
nCurrent RN license in the state of Maryland.
nMinimum of 1 year of nursing experience, preferably in home care or community health.
nStrong assessment and critical thinking skills.
nExcellent communication and interpersonal skills.
nAbility to work independently and manage time effectively.
nCompassionate demeanor with a passion for patient care.
nCurrent CPR and First Aid certifications.
nReliable transportation for travel to client locations.
