Registered Nurse (RN) – Home Care
Affirmed Home Care is seeking compassionate, skilled, and dedicated Registered Nurses (RNs) to provide exceptional one-on-one care to clients throughout New York City and Long Island.

Starting at $55.00 per hour
Performance-based incentives available
Skilled Nursing Visit Rates
IV/Infusion Visit: $125 per case
Wound Care Visit: $125 per case

Responsibilities
Conduct comprehensive patient assessments
Develop and implement individualized care plans
Administer medications, treatments, and nursing interventions
Monitor patient conditions and communicate significant changes
Provide skilled nursing services, including:
Wound Care
IV Therapy
Catheter Care and Management
Educate patients and families on:
Disease Management
Medication Compliance
Home Safety Practices
Maintain accurate and timely clinical documentation
Ensure compliance with all state and federal regulations
Provide compassionate care and emotional support to patients and families
Qualifications
Active New York State Registered Nurse (RN) License (Required)
CPR/BLS Certification (Required)
Minimum 1–2 years of nursing experience preferred
Home Care, Geriatric, Hospice, or Related Clinical Experience Preferred
Strong clinical assessment and critical thinking skills
Ability to work independently while maintaining communication with the clinical team
Excellent interpersonal and communication skills
Experience with Electronic Health Records (EHR) preferred
Valid Driver's License and Reliable Transportation Preferred
