Job description
Deliver critical care and support to patients in their homes while enjoying a structured full-time schedule as a Home Health RN. This 13-week contract is ideal for professionals seeking to make a meaningful impact within a compassionate, dynamic home health environment.
Schedule:
Monday Friday, 8:00 AM 5:00 PM
Field-based: travel to various patient homes within the assigned territory
Caseload: 30 points per week (typically 35 visits per day)
Essential Qualifications:
Active, unrestricted RN license in the state
At least 1 year of recent Home Health experience (field-based strongly preferred)
Proficient in Home Care Home Base (HCHB) electronic documentation
Skilled in OASIS assessments and documentation
Reliable transportation, valid drivers license, and auto insurance
Key Responsibilities:
Conduct in-depth OASIS assessments and develop personalized care plans
Deliver skilled nursing interventions, such as wound care, IV therapy, and medication administration
Monitor patient conditions, communicate changes to the broader care team, and coordinate ongoing treatments
Educate patients and families on diagnoses, medications, and preventive strategies
Accurately document all care in HCHB and ensure regulatory compliance
Cooperate with interdisciplinary colleaguesPTs, OTs, MSWs, and HHAsfor integrated patient care
Attend case conferences, team meetings, and maintain professional education

$2,057 - $2,176/WeekEstimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
