Travel RN Hospice Case Manager jobs in Central Vermont, VT let you serve the facility, a community-focused home health and hospice agency with a supportive interdisciplinary team. You will manage hospice cases, provide compassionate care in home settings, coordinate services, and document in McKesson or Netsmart electronic medical record (EMR) systems.To qualify, you need an active Vermont or compact Registered Nurse (RN) license, Basic Life Support (BLS) certification, and at least 2 years of hospice case management experience1. Experience providing hospice care in home settings and rural travel is required.
