Overview
Asa Registered Nurse Hospice Case Manager with Spero Hospice, you will provide compassionate care at pivotal times in patients' lives. Responsibilities
Coordinate and manage hospice care plans for assigned patients
Assess patient needs and develop individualized care plans with interdisciplinary team
Provide compassionate direct patient and family support, education, and advocacy
Collaborate with physicians, social workers, and spiritual care providers
Ensure compliance with regulations, documentation, and quality standards
Qualifications
Active RN license in relevant state
Hospice or palliative care experience preferred
Strong communication, assessment, and care coordination skills
Ability to work collaboratively within an interprofessional team
Commitment to patient-centered, holistic care
