Job Description:
Compiles data, tracks outcomes, and makes recommendations for process improvement.
Develops and maintains relationships with physicians, post-acute providers, therapists, patients, and families.
Collaborates with care management, physicians, and clinicians to develops discharge plans requiring post-acute services.
Ensures seamless transitions across the continuum of care.
Educates patients and families about post-acute care services and expectations.

Competent with Microsoft Office (Required)
Possess strong organizational skills with the ability to prioritize, manage and complete multiple tasks and projects within specified timeframes. (Required)

Teaching and counseling patients, families and staff as needed. (Required)
Strong communication skills. (Required)
Education:
Associate's of Nursing (Required)
Bachelor's of Nursing (Preferred)
