About the Role
Samash Holistic Home Care LLC is looking for a compassionate and dedicated Registered Nurse to join our growing team in Medford, NY. Responsibilities
Conduct comprehensive patient assessments and develop individualized care plans
Administer medications, treatments, and skilled nursing procedures in a home care setting
Monitor and document patient health status, progress, and changes in condition
Coordinate care with physicians, therapists, and other healthcare team members
Educate patients and family members on health management, medications, and holistic wellness practices
Ensure compliance with all state, federal, and agency regulations and standards of care
Provide wound care, IV therapy, and other specialized nursing interventions as needed
Requirements
Active and unrestricted Registered Nurse (RN) license in the state of New York
Previous home care, community health, or clinical nursing experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Reliable transportation and valid driver's license for home visits in the Medford, NY area
CPR/BLS certification required
Compassionate, self-motivated, and able to work independently in a home care environment
