Job description
Job Summary
We seek a highly skilled and experienced Registered Nurse with specialities in Medical/Surgical. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are critical. At least 2 years of experience and exceptional skill is required.
Responsibilities
Provide skilled nursing care to patients in a Medical/Surgical setting.
Conduct patient assessments and monitor patient conditions.
Administer medications and treatments as prescribed.
Develop and implement individualized care plans and treatment planning.
Collaborate effectively with interdisciplinary healthcare team members.
Manage case coordination and hospice care for patients with serious illness.
Communicate clearly with patients, families, and healthcare providers.
Required Skills & Certifications
Active Registered Nurse license in the state of practice.
Minimum of two years of hospital, home health, or hospice care experience.
Clinical assessment, medication administration, and treatment planning skills.
Excellent communication and interpersonal skills.
Bachelor's degree in Nursing or related field.
Driver's license, car insurance, and automobile for field work.
Preferred Skills & Certifications
Experience in ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room settings.
Advanced certifications relevant to Medical/Surgical nursing.
Additional training in hospice or palliative care.
Special Considerations
Ability to work in fast-paced, high-acuity environments.
May require on-call availability depending on unit needs.
Work may include hospice and home health care settings.
Scheduling
Work schedule may include day, evening, or night shifts.
Shift assignments determined by department needs and staffing requirements.
Full-time or part-time opportunities available based on operational needs.
