Job description
VNS Health is seeking a dedicated Registered Nurse to provide per diem personal care and nursing services to patients in their homes and community settings across New York City. In this role, you will perform comprehensive assessments, develop and update care plans, and deliver skilled interventions including medication administration, wound care, and symptom management. You will educate patients and caregivers on disease management and self-care, support activities of daily living, and collaborate closely with an interdisciplinary team.
