Bilingual Spanish Field Registered Nurse – Home Care
Brooklyn, NY – Bushwick
Monday–Friday | 35 hours/week
Field-based | 5 visits per day
️ Spanish bilingual required
A Brooklyn-based home care organization is looking for a bilingual Spanish Registered Nurse to join its team. This is a field-based role focused on assessing clients, coordinating care, developing and monitoring plans of care, and supporting patients and their caregivers in the home.
What you’ll do:
Complete comprehensive in-home nursing assessments and reassessments
Develop, implement, and monitor individualized plans of care
Coordinate services with clients, families, physicians, care providers, and community partners
Provide ongoing patient/caregiver education and support
Make recommendations regarding continuation or modification of home care services
Supervise Home Health Aides and assign duties based on client needs
Respond to changes in condition and emergencies as needed
Maintain accurate and timely clinical documentation
Coordinate with internal and external healthcare teams to ensure continuity of care
Participate in quality improvement initiatives and staff meetings
What they’re looking for:
Current New York State RN license
Bilingual Spanish required
Home care or CHHA experience strongly preferred
2+ years of home health experience preferred
3+ years of relevant nursing experience
BLS certification
Strong assessment, organization, communication, and care coordination skills
Comfortable working independently and traveling to clients’ homes throughout NYC
Service Area:
Brooklyn (primarily Bushwick)
Schedule & Benefits:
Monday–Friday, 35 hours/week
Flexibility to work weekends when needed
Approximately 5 visits per day when in the field
