RN Navigator – Mercy

In this Healthcare & Medical Services role, you will guide patients through complex care pathways, facilitate timely referrals, and collaborate closely with interdisciplinary teams to reduce barriers and improve outcomes. You’ll educate patients and families, support discharge planning, and leverage EHR tools to track progress and quality metrics.
