Registered Nurse (Per Diem) - Home Health

As a Registered Nurse (Per Diem) on our Home Health team, you will play a vital role in delivering high-quality, patient-centered care to seniors in the comfort of their own homes. Our team is dedicated to providing exceptional care and support to our clients, and we are seeking a compassionate and skilled RN to join our per diem team.
Responsibilities:
Provide skilled nursing services to clients in their homes, including assessing patient needs, developing and implementing individualized care plans, and educating patients and families on self-care techniques
Collaborate with other healthcare professionals, including physicians, therapists, and home health aides, to ensure comprehensive care and support
Document patient progress and communicate with the interdisciplinary team to optimize patient outcomes
Participate in patient scheduling, care coordination, and other administrative tasks as needed
